Lincoln Psychiatric Group patients can handle their paperwork and requests online through our new patient portal at my.lpglincoln.com.

In the portal you can:

  • Request medication refills
  • Sign your Telehealth Consent
  • Complete or update your New Patient Form
  • Sign Release of Information forms
  • Take the PHQ-9 and GAD-7 check-ins and see your history
  • View, print or download your forms

Getting started: go to my.lpglincoln.com and select Register to create your account with your email address. If you already have an account, just log in.

We will no longer accept refill requests or forms through [the old method, e.g. phone message, fax, email].
